The Program operates under strict admission ceilings and rules set by the Graduate Division. Applicants are therefore encouraged to see that all application forms, transcripts, and supporting materials are sent to the appropriate persons before the deadline date.

- Contact Information for Referees: The names and current addresses of three people who are in a position to evaluate your potential as a MA/PhD candidate, and whom you have asked to submit letters of recommendation on your behalf.
- All applicants are required to take the Graduate Records Examination (GRE). GRE scores should be no more than five years old. Students need to request that the scores be sent electronically to the UCSF Graduate Division.
- We will also accept MCAT scores. Applicants can send a hardcopy directly to the department.
- All foreign students whose native language/mother tongue is not English must also take the TOEFL examination. The scores may be entered online but the official reports of TOEFL scores must be sent by the testing organization directly to the:

- A Statement of Purpose: a statement (4-5 pages in length) indicating your background, general career goals, and plans for study in History of Health Sciences (must be written in English).
- Written Work: An example of your written work, such as an article or term paper, or thesis chapter.

- Letters of Recommendation: Letters of recommendation can be uploaded by the recommendor, or sent directly to the Department no later than two weeks after the February 1 deadline.
Transcripts: Official transcripts of graduate and undergraduate records should be sent directly to the Department and should arrive no later than two weeks after the application deadline.
